Accessing your Nationwide 401(k) Account
Alpine Homecare, LLC

You can begin accessing your 401(k) account immediately by using either the Internet or a toll-free 800 number. In order to use these two systems you will need to set up your account using the following instructions:


Internet Access
Go to https://www.nationwide.com/login
1.   Select “Sign Up” under the Log In button
2.   Type in your First Name, Last Name, Date of Birth, Zip Code and Social Security Number
3.   Type in 063-85052 as your “Account/Policy Number”
4.   Click on “Continue”
5.   You will then be asked to create a Username and Password (case-sensitive)
6.   Click on “Continue”
7.   Select 3 security questions, one from each drop down box
8.   Click on “Continue”
9.   Enter an email address you would like to have assigned to your account
10. Click on “Continue”
11. Select the box to agree to the Electronic Service Agreement
12. Select “Send”
13. You may now log-in to your account


Inquire System (toll-free number)
1.  Call 1-800-772-2182
2.  Enter your Social Security Number
3.  Enter your PIN Number = 1234 on first-call only
4.  System will then ask you to re-enter a new PIN Number
5.  Follow prompts to review your account

You can use either of these systems as often as you like to check your account balance, change your allocations or exchange money from one mutual fund to another. You have access to a number of retirement planning tools under the “Planning” tab to help you better understand the impact your 401(k) will have on your retirement.

In addition to these systems, you can contact the Nationwide participant call center at 877-588-6724. Call center representatives can assist you with questions including (but not limited to) web and inquire system access, account balance and deposit verification, status of requested payments, and unit value calculations.
